What it shows

Medicare hospices and home health agencies (HHAs) grouped into networks: agencies that share an owner, a person with operational control, or a practice address in CMS ownership and enrollment files. Officers, directors and managing employees are drawn inside networks but never join two networks. Very common links are also excluded from joining: people or organizations on many enrollments (chains, consultants), holding companies on more than five enrollments, executive-suite / virtual-office addresses, and addresses with many agencies.

What it does not show

Nothing here is an allegation of fraud or wrongdoing. Shared owners and addresses are normal in health care: families, chains, management companies and shared office buildings all produce them. A link means only that a name or address appears in the listed CMS release, and links in one network or one path may date from different releases.

Identity. Each person or organization node is one CMS associate ID (the PECOS ID in the owner files), not a name. Different IDs stay separate even when the names match, with one exception: two IDs with the same name are merged when both are listed on the same agency (people: same first, middle and last name, with the full middle name, not just an initial; organizations: same full name including the legal form). Same-name people without a matching full middle name stay separate even on the same agency (they can be relatives), and the node notes the other ID. Such a node lists every ID it holds and the agencies they share. Every other ID with the same name anywhere on the site is listed on the node, with links to its networks, the number of agencies and co-listed people, organizations and addresses the two IDs have in common, and their middle names where CMS gave one. These matches do not join networks. Find connection uses them only when "also step between same-name CMS IDs" is ticked, and marks each such step as unconfirmed. Some IDs were listed under different names across releases (spelling, nickname, a changed surname, a renamed company, and occasionally names that look like different people); the node lists every name with the releases it was used in, and each link record shows the name CMS listed. An incorrect or reused CMS ID can still produce an incorrect link.

People shown. Names come from the CMS owner files, which list owners, people with operational or managerial control, officers, directors and W-2 managing employees. A managing employee, officer or director listing does not imply ownership, and none of these listings implies wrongdoing. The "authorized official" is the person named in the agency's NPPES record and does not by itself mean ownership or control.

Addresses are matched after standard abbreviation (STREET = ST, NORTH = N, "1711W TEMPLE" = "1711 W TEMPLE"); a number that belongs to the road name (LOOP 410, STATE ROAD 7, AVENUE U) is not read as a suite. In addition, 176 spellings of the same building (same ZIP code and house number) are merged: 130 where the street name is the same once a missing street type or the direction is ignored ("4444 CORONA" = "4444 CORONA DR"), 43 near-identical spellings ("DUBLUN" = "DUBLIN"), and 3 joined through another spelling. Spellings with different street types (DR and PL) or opposite directions (N and S) are never merged directly. Each merge was checked with the U.S. Census Bureau geocoder: 154 resolve to the same address, 5 have only one spelling in the Census data, 16 have neither and remain unverified, and in 1 (8130 Baymeadows Way W, Jacksonville) the Census matched one spelling to a nearby street although both spellings name the same street. An address node lists its merged spellings, and each link shows the spelling CMS used. Names and addresses are displayed in title case for readability; CMS lists them in capitals, and the link records and source rows show them exactly as CMS spells them. A shared address joins two agencies only if CMS lists both there in at least one common release; an agency listed at the same address only before or after the other (a later tenant of the same suite, for example) is shown at the address but not joined through it. Addresses without a suite number that were checked to be multi-tenant office buildings, and shared office centers whose tenants all use the center's suite number, are shown but do not join networks. A building whose agencies use different suites gets its own building node, labeled with how many agencies CMS has listed there; different suites are usually separate tenants, so building nodes do not join networks either. Revoked agencies are shaded by the reason in the CMS Revocation Extract: abuse of billing privileges (42 CFR 424.535(a)(8)), affiliation ((a)(19)), or other grounds. A matched practice address may be a residence or shared premises and does not establish a personal relationship or shared control.

"Not in latest file" means the enrollment is missing from the newest CMS enrollment release and has no revocation row. It does not mean the agency closed. "Revoked" repeats the CMS Revocation Extract of 2026-10-21; the reason shown is the CMS reason code text. A revocation is a CMS administrative action, not a criminal finding; it can be appealed and may later be reversed, and a revocation of an agency is not a finding against any person listed on it. Check the current CMS data before relying on it.

Data

All sources are public U.S. government data. Every link lists the CMS records behind it: one entry per distinct row (role, ownership %, association date), with every quarterly release it appears in. Each release links to the matching rows in the CMS data API (JSON), filtered by enrollment ID and associate ID. Agency fields link to their public source: the Care Compare page for the CCN, the NPPES page for the NPI, the revocation row, and the payment row. The data may contain errors from the source files or from address and ID matching. A missing "revoked" marker does not mean an agency was never sanctioned: only the CMS Revocation Extract is checked.

Report suspected fraud

This site does not take or forward tips. If you have information about Medicare or Medicaid fraud, waste or abuse, report it directly to the government:

Include the agency name, its CCN or NPI, and what you observed. A network or link on this site is not by itself evidence of fraud.

Corrections

If something is wrong, please report it to lasushin@proton.me, with the network number and the name or link concerned. Requests to correct or remove an entry are answered there. Corrections to the underlying records must be made with CMS (PECOS).
